Elderly Care Application Process

  • Signed application form submitted by the family
  • Completion of assessment information
  • Submission of medical information that will assist in the individualized program plan
  • Provide living will with DNR order if applicable
  • Payment of registration fee

Eligibility Criteria for Elderly Care

Appropriate clients are those who cannot be safely left alone at home and live with someone who either works outside the home or needs respite. Client admission will be viewed as 30-day trial for both parties after this time the decision will be made about client’s continued attendance.

Cancellation Policy

Cancellation of attendance should be made at least 24 hours in advance whenever possible. If cancellation is due to a sudden illness, the Center should be informed at least one half hour prior to the time the client is scheduled to arrive at the Center. Messages may be left on our voice mail at any time of the day.
